Liability waivers & participant forms
Safety starts on paper. Every participant completes these forms before live fire. Download, read, and bring them signed — or arrive 15 minutes early and we'll fill them out together.
Required for every participant
Release of Liability, Assumption of Risk & Indemnification
Covers classroom instruction, dry fire, live fire, range visits, rental firearms and equipment, and any transportation arranged for the session. Must be signed before any live fire.

Participant Registration & Safety Information
Contact and emergency information, shooting experience, goals, and any health or accessibility details that affect safe instruction. Kept confidential and used only for instruction and emergency response.
Reviewed and initialed on site
Range Safety Rules & Code of Conduct
The core firearm safety rules and range conduct expectations. Read it in advance — we'll go over it together and you'll initial each rule before we head to the line.
Minors & guardians
Participants under 18 must have a parent or legal guardian sign the release and the safety-rules acknowledgment, and the guardian must be present for the session.
